Patients and editing

This page is Horizon. Lite can show patients your organization allows. It does not use Horizon’s Ready lock.

Patient information is one record. You can reach it from a study or from the patient profile. Both places edit the same data.

Study card or patient profile

From a study, a patient card shows the demographics for that encounter. Use it for a quick check without leaving triage. The patient profile is a dedicated page with demographics, free-text medical history, a locked MRN, and change history. Go there when you need more than a glance.

What you can edit

Display name, date of birth, and sex are the usual demographic fields. Free-text medical history sits on the same surface. It is optional, audited, and kept when imaging re-ingests the patient with fill-only rules. MRN is system-managed and read-only. Resource UUIDs stay out of the product UI.

Save writes immediately. Validation errors show on the form. Changes appear in change history with old and new values highlighted.

Structured FHIR-style sections such as conditions and allergies are not full product editors here. Integrations may feed them separately.

Permissions

Editing needs the right site permission. If you are read-only, forms may still appear while Save stays hidden or disabled. The same rule applies on study and profile views.

When demographics lock

Sometimes a banner says patient demographics are locked. That is intentional. When another study for the same patient is already Ready, Horizon freezes demographic edits so a report that is about to start does not race with identity changes.

While locked you can still read fields, browse history, and keep imaging and reporting work that does not need demographic edits. To unblock urgently, follow site policy. Sometimes that means moving the other study out of Ready, fixing the patient, then marking Ready again. Some organizations route corrections through admin or HL7/FHIR instead.

Which view to use

A quick check before marking Ready belongs on the study page. Full history cleanup and browsing documents outside one study belong on the patient profile.
